Data Availability

All protocols were reviewed and approved by the NCHS Ethics Review Board with the U.S. Department of Health and Human Services Policy for Protection of Human Research Subjects before NHANES was implemented (32). Furthermore, this study was acknowledged by the Johns Hopkins University Institutional Review Board as Not Human Subjects Research (IRB00404102) and as a Quality Improvement project. Formal consent was not obtained as the dataset had all patient identifiers removed.
