Data Availability

Individual participant data, after having been anonymized, will be made available, along with the study protocol, statistical analysis plan and informed consent form template. Data will be available beginning 3 months and ending 5 years after publication of the article to researchers whose proposed use of the data is approved by the study sponsor. Proposals should be directed to the corresponding author and requesters will need to sign a data access agreement.
