Data for this study were collected and analysed under the National Disease Registries Directions 2021, made in accordance with sections 254(1) and 254(6) of the 2012 Health and Social Care Act. Further ethical approval for this study was not required per the definition of research according to the UK Policy Framework for Health and Social Care Research. The data are available to those that have the legal basis to access it, either through the Data Access Request Service (https://digital.nhs.uk/services/data-access-request-service-dars) or partnership with National Disease Registration Service (NDRS). The protocol was approved by the joint NDRS project board reference PPF1920_027.
