A waiver of ethical approval was received for this study from the PHE Research Support and Governance Office. Ethical approval for this study was not required as the collection of information on patients quality of life is part of the statutory tasks and routine surveillance activities of Public Health England and its legal successor, the UK Health Security Agency.
