Anonymized datasets from the National Hospital Ambulatory Medical Care Surveys (NHAMCS) and National Health and Nutrition Examination Survey I (NHANES I) are publicly available from the National Center for Health Statistics of the U.S. Centers for Disease Control and Prevention. Data from the Boston study may be available from the authors upon reasonable request, subject to privacy restrictions.
